Admissions

We are now accepting enrollment applications for the 2013-2014 school year.  Our application is available below for you to download.  You may also pick up a form in the school office or call us and we will gladly mail one to you.

The application is available in two formats:

• Fill out electronically & Print

  This document has the fill-in fields set up for you to fill in using Adobe Reader on your computer.  Once the document appears on your screen, follow any instructions from Adobe Reader. As you fill in the document, you may continue to tab from field to field to fill it out entirely.  Then print and return to the school office.  You may also save the file on your computer and fill it in later.

  Tips: The document must completely download before the fill-in fields will be available. From Adobe Reader you can click the Highlight Fields checkbox in the upper-right hand corner of the screen to add/remove highlighting to the fields that need to be filled in. You are only allowed to save the unfilled document to your computer.

  
  
• Print & Fill Out by Hand

  This document doesn't have fill-in fields.  Once it appears on your screen you may click the Print icon and print it on your printer.  Then fill out the form by hand and return it to the school office.
